Explaining the Process
All applicants will be required to complete an application for services.
This can be arranged by contacting the office. We can schedule an in-office visit or home visit, whichever is most convenient for you.
The Rehabilitation Counselors will receive your application, explain the program and services available and will collect the information we need to determine eligibility for this program.
Based on current regulations, the Rehabilitation Counselors are given 60 days to make an elgibility determination.
The Rehabilitation Counselors will review the information and decide whether they recommend an eligibility determination.
If you are closely related to the Rehabilitation Counselor that you are working with or if you would prefer to work with another Counselor for any reason, please contact the Program Director.
The Director will accept your request with no need to provide an explanation and will reassign your file to another staff member. |
