Annuals and Labs Due as of 6/1/2010 Below is a list of all the labs and physicals due for each group. Explanations: Annual Assessment – Annual Assessments require a lab and physical. The physical is conducted at the Balancing Our Lifestyle building with John Nicholson, RN, PHN, and Tracey Brower, FNP. The physical should take approximately 30 minutes. You must report to lab to have a fasting blood sugar, A1C, cholesterol check and albumin creatinine or AC Ratio (urine test) test conducted. Again, this is a fairly simple lab work and should not take more than 10-15 minutes. **Please NOTE: We are no longer requiring the 2 hour Oral Glucose Tolerance Test (orange sugary drink). Mid-Year: If you are due for a Mid-Year all you need is a fasting blood sugar and A1C. This is a quick and easy lab procedure, should take less than a couple minuets for a couple vials of blood.
